As a healthcare contractor, you give everything to your patients, long hours, demanding shifts, and often high-pressure environments. But when it comes to protecting your own rights and financial security, things aren’t always so clear.

One area that causes confusion is holiday pay and sick pay. Do you get them as a contractor? How much are you entitled to? And who’s responsible for paying you if you can’t work?

This guide breaks it down simply, covering:

  • How holiday pay works for healthcare contractors
  • What happens if you need time off sick
  • The role of your umbrella company
  • Why compliance matters for your pay and rights
  • Why healthcare professionals trust Danbro Umbrella

Are Healthcare Contractors Entitled to Holiday Pay?

Yes. If you’re employed via an umbrella company, you’re legally entitled to statutory holiday pay, the same as permanent employees.

Your entitlement is 5.6 weeks per year, which includes bank holidays.

How Much Holiday Pay Will You Get?

Holiday pay is based on your average weekly earnings. For contractors on a pro-rata basis, depending on how many weeks you’ve actually worked.

A common rule of thumb is 12.07% of your gross pay based on working 46.4 weeks a year.

Example:

  • Weekly pay: £500
  • Holiday pay entitlement: £500 x 12.07% = £60.35 per week
  • Annual holiday pay (based on 52 weeks): £3,138.20

These are your earnings, saved to support your well-deserved time off.

How is Holiday Pay Paid?

Umbrella companies usually offer two options:

  1. Accrual Method – Holiday pay is held back and paid when you actually take time off
  • Helps you budget for unpaid time off
  • Requires planning to make sure you use your entitlement
  1. Rolled-Up Method – Holiday pay is included in your weekly wages
  • Gives you immediate access to all your earnings
  • Can be misleading – you may forget that your holiday entitlement has already been paid

Both methods are legal, but transparency is crucial. Always check your payslip to understand how your umbrella company handles holiday pay.

Sick Pay for Healthcare Contractors

Working in healthcare is demanding, and sometimes illness or injury means you can’t attend shifts. If you’re employed by an umbrella company, you may be entitled to Statutory Sick Pay (SSP).

Statutory Sick Pay Rates 2025

To qualify, you must:

  • Be off sick for at least 4 consecutive days
  • Earn at least £123 per week on average (before tax) [as of 2025/2026 tax year]
  • Provide medical evidence (such as a GP fit note) if you’re off for more than 7 days

Unlike permanent NHS staff, contractors typically don’t receive enhanced sick pay – SSP is the standard entitlement.

Example:

If you normally earn £500 per week, SSP would provide just £118.75 – a sharp drop in income. This makes it vital to understand your rights and plan ahead.

Who is Responsible for Paying Sick Pay?

If you’re working through an umbrella company, they are your legal employer. That means:

  • The umbrella company (not your agency or NHS trust) process and pays SSP
  • They handle all your statutory rights, including holiday pay, pension, and sick pay

This is why choosing a compliant, supportive umbrella company is essential.

Why Compliance Matters

Not all umbrella companies operate fairly. Some may:

  • Withhold or “disappear” unclaimed holiday pay
  • Use confusing payment structures
  • Make unlawful deductions

By choosing an FCSA-accredited umbrella company, you protect yourself. Accreditation means the umbrella company has been independently audited for compliance, transparency, and ethical practice.
